What should you bring to your appointment at BCGI
We want to answer as many of your questions at your visit.
NOTE: We do not see patients under the age of 18 years old.
Please prepare for this by bringing
- Health Insurance Card / Information
- A friend or family member, if needed, to take notes/help with questions
- A list of all medications you are taking
- A list of your questions / concerns
- Copies of records from doctors related to your visit with us.
- Completed FORMS below
